First of all, that would be a 4-card combo involving three copies of the same card that also requires you to have three extra cards available as discard fodder.

Second of all, Twin Swords doesn't work that way. It grants the ability to attack twice, not the ability to attack one additional time. Having multiple copies of Twin Swords equipped still only gives you two attacks per Battle Phase.
