What the title says.
Remember playing FF8 and finding out how to unlock a GF, so went and did it as instructed not totally sure what to expect, when this happened
*this is a video of some other dude (being a dick), so the impact is lost with that and not being on a full screen TV with max volume and stuff*
When that King Tonberry suddenly appeared I had to pause for about five minutes... to recover from the shear heart attack it had just given me!

I'd expect no less from the hero of an epoch.
Like Likes. Just seeing them, hearing the sound they make, or even listening to those slug things in the Fire Temple that make sounds vaguely resembling them, are enough to terrify me.

Hand Masters from OoT. When you least expect it, they cast a shadow and grab you, then take you to the beginning of the dungeon. Not only was that frightening, but it was annoying.

For continuous tense, edge of seat kinda nerves that could be shot at any second, Sniper Elite on PS2 was pretty horrendous on the nerves.

Don't forget the Silent Realms. That freaking change of atmosphere when you trip up and the Guardians chasing you seriously nearly gave me a heart attack the first time.
