If your opponent destroys a monster you control (by battle or card effect): You can discard this card; destroy 1 card on your opponent's side of the field. You can banish 3 cards from your Graveyard; Special Summon this card from your Graveyard. You can only activate 1 of these effects of "Shinigami Belladonna" once per turn.

I believe the effect should be written this way:

"If your opponent destroys a monster you control (by battle or card effect): You can discard this card; destroy 1 card your opponent controls. You can banish 3 cards from your Graveyard; Special Summon this card from your Graveyard. You can only use 1 "Shinigami Belladonna" effect per turn, and only once that turn."

 

To be honest, I'm not entirely sure how this card will be used. It is not a hand trap (it would've been better if it was), and even if it was, its effect is not incredibly powerful compared to other hand traps. The condition is not too situational but it could've been better if it negated the destruction, but that might end up being too good. But I'm more concerned with its revival effect - banishing 3 cards to revive a Level 4 monster is quite a minuscule cost especially when you consider how easy it is to dump cards in your Graveyard in today's meta. For comparison, consider Zephyros the Elite whose effect can only be used once per Duel.

Its first effect is pretty decent imo. It somewhat reminds me of the Trap "Bad Aim", since it lets you destroy a card on the field after your opponent does something.

 

The second effect makes it a good Material Synchro and Xyz Summons. Like the above comment said, though, it's too unrestricted. Maybe if it were banished when it leaves the field after being Summoned in that way, then it could be more balanced.

Maybe I could alter the destruction effect, but given how the Special Summoning effect is, I think I may leave it as is.

 

That's not really the best comparison. Zephyros's effect is once per Duel for a few reasons. For one, once per Duel was a clause that was being used more often in that generation (Glow-Up Bulb, Spore, ect.). And even disregarding that, all you do to Special Summon it is return a face-up card. You can still use that returned card. You can't reuse any of the cards you banish for this card's effect.
